21M with low BMD for age, normal labs/TBS/VFA, sports fractures, and pathogenic ORC6 variant. What should I ask next?

Hi everyone,

I’m a 21-year-old male trying to understand a low bone density workup. I’m already seeing endocrinology and have follow-up planned, but I’d appreciate advice from anyone with young-onset low BMD, genetic bone findings, or similar DXA results.

Background

  • Male, 21
  • Height around 6'1" to 6'2"
  • Weight usually mid-160s
  • Lifting consistently 4+ days/week with progressive overload
  • High-protein diet
  • Trying to maintain calcium and vitamin D intake
  • No smoking
  • No obvious secondary cause found so far

Fracture history
I have had prior fractures, but they were sports-related:

  • Dec 2021: right humerus and ulna fracture during a sports-related fall, required surgery/hardware
  • 2023: right finger fracture during sports

I’m not sure whether these should be interpreted as fragility fractures or traumatic/sports fractures. My endocrinology note lists osteoporosis/low BMD with history of right humeral/ulnar fracture and right finger fracture, unclear etiology.

  • Left femoral neck: BMD 0.711, Z-score -3.1

  • Lumbar spine L1-L4: BMD 0.831, T-score -2.4, Z-score -2.1
  • Left femoral neck: BMD 0.596, T-score -2.5, Z-score -2.5
  • Left total hip: BMD 0.756, T/Z -1.8
  • Right femoral neck: BMD 0.679, T/Z -1.8
  • Right total hip: BMD 0.751, T/Z -1.9
  • Mean total hip: BMD 0.754, T/Z -1.8
  • TBS AP spine: 1.322, interpreted as normal microarchitecture
  • VFA: no obvious vertebral deformities/fractures
  • FRAX not reported because I’m under 50

Important caveat: the 2025 and 2026 DXAs were done on different machines/facilities, so I know raw BMD values may not be directly comparable. The 2026 scan was treated as a baseline on that machine.

Most common secondary causes seem negative/normal.

24-hour urine calcium

  • 213, interpreted as normal

So far, there doesn’t seem to be evidence of vitamin D deficiency, hyperparathyroidism, thyroid disease, celiac disease, kidney disease, calcium wasting, abnormal testosterone, or major metabolic abnormality explaining the low BMD.

Genetic finding

Genetic testing showed a pathogenic ORC6 variant. My endocrinologist noted that ORC6 mutations can be associated with Meier-Gorlin syndrome, but I don’t have obvious classic clinical features. I’m tall, around 6'2", and I don’t have obvious microtia or patellar abnormalities as far as I know.

The note said ORC6-related Meier-Gorlin syndrome is typically associated with developmental issues in bone/tissue growth and delayed mineralization rather than progressive acquired bone loss. Additional genetic testing/referral was ordered.

Current plan from endocrinology

  • No medication for low BMD right now
  • Repeat DXA + TBS in about one year
  • Continue age-appropriate protein, calcium, vitamin D, and physical activity
  • Follow up with genetics/testing

Has anyone seen a similar case: young male, low BMD/Z-scores, completely normal labs, normal TBS/VFA, sports fractures, and possible genetic/developmental explanation? What did your doctors do, and did anything help improve or stabilize BMD?

Lowest reasonable car insurance coverage for a 21 y/o med student with a 2023 BMW 540i?

Hey everyone, looking for practical advice on what the lowest reasonable car insurance coverage would be for my situation. I’m not trying to run bare minimum because I know that’s reckless, but I also don’t want to overpay for coverage that doesn’t make sense.

Details:

I’m 21 years old, a medical student, and I don’t really have any significant assets right now. I have good health insurance through UnitedHealthcare Choice Plus. I’m in Tampa, Florida, and the car is a 2023 BMW 540i xDrive. It’s paid off/no loan. The car is worth around the low-$40k range. I have about $2,000 cash readily available, so a higher deductible is not a huge issue. I do have one prior at-fault accident from 2023 showing on CLUE.

I want to keep comprehensive and collision because I don’t want to self-insure the whole value of the car, but I’m trying to figure out reasonable liability and UM limits.

The Progressive quote I got for regular driving coverage was about $1,620 paid in full for 6 months after they pulled reports, or about $270/month effective.

That quote was for roughly:

Liability BI/PD: $50k/$100k/$100k

Uninsured Motorist BI: $50k/$100k non-stacked

PIP: $10k basic, $1k deductible

MedPay: $500 per person

Comprehensive: $1,000 deductible

Collision: $1,000 deductible

Rental: $40/day

Roadside: no/optional (I already have BMW roadside coverage)

Progressive Vehicle Protection: no

I also looked at lower/budget setups like:

$25k/$50k/$10k liability

$10k/$20k UM or no UM

$500–$1,000 comprehensive deductible

$1,000–$2,000 collision deductible

My questions:

For someone who is young, in school, and has basically no assets right now, is $25k/$50k bodily injury and $10k property damage too low?

Is $50k/$100k/$100k liability a smarter minimum, even if I don’t have much to sue for right now?

How should I think about future earning potential as a medical student? Does that make higher liability more important?

Does having good health insurance make it reasonable to carry low UM coverage, or is UM still important in Florida?

Is $1,620/6 months for the $50k/$100k/$100k liability + $50k/$100k UM setup reasonable for my profile?

What would you personally consider the lowest reasonable setup for a 21-year-old in Florida with this car?

I know nobody here is my lawyer or insurance agent. Just trying to get a realistic sense of what’s financially responsible versus overkill for a broke med student with a newer BMW.
