How to Request a VA Waiver or Relief

Time Limit: You generally have 1 year from the date of your first debt letter or copay bill to request a waiver.

Required Form: Fill out a Financial Status Report (VA Form 5655) to demonstrate your income, assets, and monthly expenses.
