Prolonged/extended release methylphenidate - how late is too late to take it?

It’s Sunday today and I received my prescription yesterday afternoon for 18mg Affenid prolonged release. It says it lasts 10-12 hours, and to take it with breakfast.

I woke up at 9 since it’s the weekend (I’m usually up for 8 on weekdays) and I’m currently making breakfast, so will probably eat and take it by half 9.

Does anyone have experience with a time they took it that was too late, and interfered with their sleep, etc?

The instructions say it can cause insomnia, but also that it might make you drowsy, so kinda conflicting!

Reapplying SPF in the evening if leaving the house?

So when I get home from work at 5-5:30 ish, I usually go straight for a shower to get all the work grime off me (also sometimes a bit sweaty due to inconsistent air con in summer).

I often go for walks on my lunch break, but sometimes I don’t and I want to go for a walk after work instead.

After I’ve showered, I usually do my PM routine and don’t reapply SPF, but in summer, the sun often doesn’t set until 9:30-10pm.

I have rosacea and use azelaic acid daily, so try to be diligent with my SPF application, but if the UV is 1 after 6pm, even if it’s still bright out, I’ll go out without SPF so I don’t have to wash my face 3 times in a day!

Just wondering what others do in a similar situation - if you go for an evening walk or leave the house etc., when it’s still bright but lower UV, do you apply/reapply SPF or go without?

Does KPRF get better over time? Suggestions for what might help? 31F still getting uncomfortable flushing

I’ve had KPRF my entire life (there are loads of pics of me at like 4/5 years old with similar bright red cheeks) and I’m sure I’ve read that it reduces in severity with age. For me, I haven’t noticed it getting better yet!

This pic was taken about 15 minutes after getting back in after a 30 minute walk. Such a low intensity exercise and yet it feels like my face is being roasted in the oven.

I also developed rosacea in my mid 20s, but that’s mostly concentrated on my nose and middle of the face (like the cheeks next to my nose), so when I’ve been exercising, my cheeks look like this due to KPRF and then my nose is just like a tomato too.

Any experiences with it getting better over time, or any successful methods of reducing the redness/burning?

I use azelaic acid for my rosacea which also helps with a lot of the texture and bumps from KP, but stuff like niacinamide makes it all worse.

Did fixing RCPD/getting botox reduce anyone’s GERD or acid reflux?

I know that while the botox is active, it’ll increase GERD symptoms (due to the sphincter being paralysed to let all the stomach acid back up when lying down, bending, etc etc) but I’m talking more about after the botox has been successful, has worn off and you’re able to burp normally. Did it improve your GERD/acid reflux, etc?

I have a small hiatus hernia (suspected to be caused by decades of RCPD) and take 15mg Lansoprazole daily.

I’m getting my botox in August and will increase my dose to 30mg for a few months while the botox is active, but I have been on PPIs for 6+ years now, and would like to not really be on them daily for the rest of my life. Especially since it inhibits vitamin absorption (I have chronically low iron) and with the increased dementia, etc., risk.

My gastro who diagnosed my hernia mentioned that it was too small to operate on, but small enough that it shouldn’t be causing that many side effects. I’m hoping that being able to relieve the gas buildup will help with the acid reflux and other symptoms I sometimes get, or get when I skip my PPI for a few days (so like shortness of breath, chest pain, nausea, acid reflux causing a sore throat at night, etc).

Anyone have any experience with this?

How long do you leave in between meds + PPIs?

I’ve been taking 15mg Lansoprazole daily since 2021 due to a hiatal hernia that causes GERD symptoms.

I usually take it first thing in the morning, or within half an hour of waking up, so I can eat breakfast about half an hour later.

I’m starting Methylphenidate shortly (my ECG has been sent off and I’m waiting for my prescription, so within the next week) but I won’t be speaking to the titration handler (?) before I start the medication, the prescription will come through, I start taking it, and then have a follow up.

For those who take stimulants and PPIs, how do you take them? I’d imagine I need to take Methylphenidate first, but do I have to wait 1-2 hours before my PPI, etc? I worry that by then my appetite might be suppressed, etc. My GERD tends to be worse when I go long periods without eating, so I try to avoid fasting etc.

Blood clots during ovulation?

So I’m 30 and have been getting regular periods again since I was about 26-27, when I came off Nexplanon after being on it for 7-8 years.

As a teenager, I didn’t really get heavy or long periods, cramps were mild, not really any hormonal issues at all. The BC was just because I got with my bf at the time (19). I’m still with him, but he got a vasectomy in 2022 so I came off BC.

Towards the last few years of my implant, I started getting ovarian cysts more frequently, so that solidified my choice to come off hormonal BC.

I’m still not on any BC today, but as the years have gone by, my periods have gradually gotten heavier. I started getting ovulation cramps a few years ago too, which is fine, but since last year I also developed spotting during ovulation and sometimes after sex. Even if there was no penetration!

Every few months, I now get blood clots during ovulation. I’m coming towards the end of this month’s ovulation and have been passing random brown blood clots for about 4 days. Not all day, but randomly after wiping or in the shower.

They’re about the size of my thumbnail, I found about 4 or 5 of them in the shower two days ago and I had one day of right sided cramps yesterday.

**I had an ultrasound earlier in the year (both transvaginal and a normal abdominal one) which didn’t find any evidence of fibroids, etc. It was after ovulation so the US tech did say that my lining was quite thick and to maybe come back after my period when it would be thinner, but from the ultrasounds, did found a very small ovarian cyst (way smaller than the ones I’ve had in the past) and wasn’t worried about it.

Any thoughts?

Can you tell if you’re in someone’s best friends list?

I currently have 4 best friends (one is my bf so we have the pink hearts emoji), but for the other 3, obviously they aren’t my #1 best friend since that spot’s already taken, but we have the 😊 best friend emoji.

Is that an indicator that they’re ONLY one of my best friends, or that I’m also in their best friends? Is there a way of knowing?

Eczema (?) patch on underarm, should I get my next IPL round or wait until it’s gone?

I’ve had this patch on my underarms for a few weeks now, only on one armpit! It’s not itchy or sore, but it feels a bit scaly. I had contact dermatitis from a natural deodorant on my underarms a few yrs ago, but I don’t use anything with baking soda in anymore, and use a natural Dove antiperspirant without any issues (seemingly).

I’m due an IPL session late next week, but this mini rash is persisting. Should I still get my IPL done around it or start Protopic and wait until that’s gotten rid of it?

Fungal? Eczema? Psoriasis? Only on one armpit and not itchy or sore!

I had something similar on both armpits a few years ago that didn’t respond to anything - including clotrimazole or other anti fungal, but was quite itchy/burning. That turned out to be contact dermatitis caused by my natural deodorant, which contained baking soda!

Fast forward a few years, this popped up a few weeks ago, no itching, burning, discomfort. Only on my left armpit and only in this small area.

Any thoughts? I use a Dove sensitive anti-perspirant stick and haven’t had any issues in the 2ish years of using it. I’ve tied using Sudocrem on it and also Cicaplast and neither has done anything.

Been taking supplements for just under 2 weeks, any ways to manage the dizziness and weakness in the meantime?

I have a ferritin of 6 without anemia, but I’m still getting a lot of low iron/anemic symptoms.

I’m sat here, not even moving, and feeling lightheaded/dizzy, exhausted and on and off heart palpitations.

I had an ECG a few weeks ago that was normal so I’m pretty sure this is just due to my iron, and my supplements (200mg ferrous sulfate) haven’t had enough time to work yet).

I’m trying to regularly drink water, I’ve eaten today too, but I still feel terrible.

Any suggestions on stuff that could help reduce these symptoms in the meantime, if such a thing exists?? I had trouble sleeping last night due to hearing my heartbeat pounding in my head. Awful stuff!

Occasional inflamed upper gums after taking retainer out in the morning

I had braces as a teenager (17-19) and then 5-6 months of Invisalign for a remaining over jet in 2021 in my mid 20s. I have a permanent wire retainer behind my front 4 teeth and don’t really have too many issues with my teeth shifting these days.

I still wear my retainer ~2 days a week, and mostly for
my teeth clenching issues. If I’ve been clenching my teeth more often (this started the first time I had braces), I’ll wear my retainer more often. I’ve been using this retainer since 2022, so it’s not new or anything.

I have a night guard for my top teeth too, but it’s SO bulky I can’t even close my mouth comfortably at night so wake up with a dry mouth. I just use my retainer as a night guard instead.

Sometimes - not every time - I wake up with my upper gums a bit inflamed. They ache a bit at first when I wake up and calm down after an hour, but you can see my gums are still a little bit swollen looking, and I took my retainers out about 3 hours ago now.

Does anyone know what could be causing this? Just the pressure from a retainer + tooth clenching? I clean my retainer properly before using it every time.

As someone who is terrible at taking care of plants, here’s my miracle ZZ plant, nearly 5 years on!

This is my oldest plant from late 2021 (the only other is a huge peace lily I got in early 2022, who is also surprisingly doing pretty well!)

I’ve had countless other plants since and like 80% of them have died - might be my ADHD, but I forget to water them for aaages and then I’m hyper aware of them for a few weeks. So they’re parched and then water boarded (so sorry 😭).

My ZZ plant does NOT gaf about any of that though. An absolute champ, he’ll take whatever he gets and is happy about it. He’s also grown SO much - when I first got him, he was 40-50cm. He’s much larger now, although I haven’t measured him for the specifics - and he’s never once been repotted. I really should, but he seems fine so I just haven’t. He has a few dead leaves I need to snip off but he won’t have a tantrum if I don’t do it immediately.

Just an appreciation post for one of THE hardiest indoor plants I’ve ever known 💕

Seb derm safe oil cleanser for double cleansing?

I have both seb derm on my face and also type 2 rosacea. The only thing that keeps them both at bay is azelaic acid, I’ve been applying it daily since February and haven’t had any issues with either condition since then.

I do get keratin plugs on my face though, despite using 20% azelaic acid, and a lot of the treatments like urea cream and amlactin I’m struggling to find that won’t trigger my seb derm or rosacea.

I’ve read that using an oil cleanser before your normal cleanser can help keep them at bay, but I’m struggling to find a seb derm and rosacea safe oil cleanser.

To remove makeup, I usually use the Clinique Take The Day Off balm, which I don’t typically have any issues since it’s only on temporarily before my regular cleanser (Avene Cleansing Foam) and I don’t wear makeup often - less than once a week.

But since I’m hoping to oil cleanse at least a few times a week, I’d rather spend money on one that’s the less likely to cause a reaction or flare up with regular use.

Anyone have any recommendations or suggestions?

Keep getting keratin plugs on face - any effective treatment options?

Pictured: A keratin plug on my inner eye corner that’s been hanging around since mid May. It started as a small black dot and then got bigger and just felt hard - not quite like a pimple or milia and wouldn’t pop, either. At the end of my tether, I managed to get it out with my nail after washing my face today, and it’s the biggest one I’ve ever had, huge! Was annoying me for over a month and left a big pore (which will calm down in a few days but still unsightly).

I hate getting these, especially in places like around my eyes. Especially since I can’t use AHAs in such a delicate area, so my treatment options for this area are even more limited than the rest of my face.

I’ve been getting keratin plugs for years, as well as having KPRF on my lower cheeks - they started on my upper arms when I was a kid/teen, but I often get them on my back, face and scalp these days too.

I also have rosacea and seb derm on my face, but they’re both pretty controlled with azelaic acid + soolantra daily.

I’d say I get more keratin plugs on my face than actual pimples - and they DO NOT leave until I manually remove them. I use azelaic acid daily and still get them, salicylic acid has never worked in the past, I haven’t found a urea or amlactin cream that I can use on my face (especially one that won’t irritate either my rosacea or seb derm).

Anyone found any success with treating these that don’t resort to scratching them out? I don’t want to increase my risk of scars and pigmentation from it. I make sure the pore’s clean afterwards and apply cicaplast or sudocrem over it, but it’s still a concern.

I couldn’t get a derm appt until early August so asking here in the meantime!
